Position Purpose:
The Home Health Nurse provides comprehensive nursing care in the community setting to help patients achieve optimum health and quality of life, especially in cases of chronic disease. This role involves the provision and management of effective nursing care to individuals or families in the community or in Patient Support Programs. The Home Care Nurse works in adherence to Axios International's Policies and Procedures, following established standards of nursing practice in accordance with local and international rules and regulations.
Duties and Responsibilities:
Service Responsibilities:
- Provide skilled nursing services in accordance with physician orders and under a plan of care. Duties include, but are not limited to:
- Telephonic follow-up consultations.
- Monitoring vital signs (blood pressure, heart rate, temperature, oximeter statistics).
- Monitoring blood glucose levels, offering teaching and support for patients and families.
- Preparing medications as per physician orders.
- Administering medications and providing instructions on various forms (injections, IVs, inhalation, oral infusions, rectal).
- Pain management, medication monitoring, and support for patients with acute or chronic health conditions.
- Education and counseling consistent with treatment and care management needs.
- Wound assessment and dressing changes.
- General nursing care.
- Knowledge of advanced foot care is a plus, but not required.
- Monitoring pain control and performing cardiac assessments.
- Supervision, health teaching, and support for families and caregivers on a regular basis.
- Assessing mental status.
- Attending and completing Pharmacovigilance Pharma and Product Training, understanding how to collect and report safety data.
